A certified copy is the document most agencies accept for a passport, a driver's license, school enrollment or benefits. Northern Mariana Islands issues it for births recorded in the state.

BirthChek is a private company. We are not a government agency and are not affiliated with any state or territory vital records office. You can order directly from the issuing office, usually for less.

Northern Mariana Islands birth certificate: the Saipan office and release affidavits

In the Northern Mariana Islands, the Health and Vital Statistics Office of the Commonwealth Healthcare Corporation on Saipan issues birth certificates to the registrant, a legal spouse, parents, grandparents, and children or grandchildren eighteen or older. Anyone else needs a notarized affidavit from the registrant, a court decree, or a government request on letterhead.

Who may request a certificate

Commonwealth law, 1 CMC 26021, allows copies for the registrant, spouse, children, parents, guardian or their authorized representative, and for others who need the record to protect a personal or property right. The Commonwealth Healthcare Corporation's request form spells out the family it accepts: the registrant, a legal spouse, parents or grandparents, and children or grandchildren who are eighteen or older. A guardian or legal counsel named in a court decree can also request, as can a government office with a memo on letterhead stating the purpose.

The affidavit to release

Someone outside that list, such as a brother, sister, aunt or friend, can still receive a certificate if the registrant signs an Affidavit to Release in front of a notary naming them. The affidavit can be used once and stays valid for thirty days from signing.

The Saipan office

The Health and Vital Statistics Office is part of the Commonwealth Healthcare Corporation in Garapan on Saipan and is open on weekdays; requests can also be mailed to its Saipan post office box. Records go back to 1946, though entries from 1946 to 1954 are incomplete.

What the request form asks

The form asks for the requester's name, mailing address, phone and email, and for the registrant's name at birth and any later name, date of birth, sex and place of birth. It also asks for the mother's name before first marriage and the father's name, with each parent's ethnicity and birthplace. A copy of valid photo ID goes with every request, and the form itself needs no notary. Certified copies carry security features against alteration and are marked "Amended" or "Delayed" where that applies.

Adoption, paternity and changes

After an adoption, a legitimation, a court paternity order or an acknowledgment, the office creates a new certificate, and the original can be inspected only by order of the Commonwealth Superior Court or as the law allows. Unmarried parents add a father with a notarized declaration of paternity. Name and sex changes are made on a Superior Court order, with a notarized application from the registrant over eighteen, a parent, or a guardian or representative holding a certified court order. Birth records become public a hundred years after the birth.

How it works when you order with BirthChek

Our Northern Mariana Islands form follows the request form's list, and an authorized representative uploads the notarized affidavit or court decree. You give the place of birth and both parents' names, upload your photo ID and sign, and BirthChek files the request with the Health and Vital Statistics Office on Saipan.
