By ticking "I agree to use electronic records and signatures", you agree that BirthChek may provide your application, the Limited Power of Attorney, receipts and every notice about your order electronically, at the email address you gave us and in your order portal.

Paper copies

You may ask for a paper copy of any record at any time, free of charge, by writing to support@birthchek.com.

What you need

A device with internet access, a current web browser and a PDF reader. Keep a copy of this notice and of the documents you sign.

Withdrawing consent

You may withdraw this consent at any time by writing to support@birthchek.com. Withdrawal applies to records not yet delivered; it does not undo documents you have already signed electronically, including the Limited Power of Attorney, whose own terms govern termination.

Your electronic signature

Ticking the acknowledgement checkbox, together with your typed full legal name, is your electronic signature under the U.S. Electronic Signatures in Global and National Commerce Act (ESIGN) and the Uniform Electronic Transactions Act (UETA), with the same effect as a handwritten signature.
