A certified copy is the document most agencies accept for a passport, a driver's license, school enrollment or benefits. Alaska issues it for births recorded in the state.
Alaska birth certificate: consent letters and the Anchorage and Juneau offices
Alaska issues a birth certificate to a parent named on the current record, the person named once fourteen, a legal guardian, an attorney or agency with supporting documents, and anyone holding a notarized consent letter from the person named or their guardian. The state offices in Juneau and Anchorage serve walk-in customers for births anywhere in Alaska.
Parents, the person named, and everyone else by consent
Alaska's request form keeps the list short. A parent named on the current certificate can order, and so can the person named once fourteen or older, who may show a current school ID. A legal guardian sends certified guardianship papers, and attorneys and government agencies send a letter on letterhead with certified or notarized supporting documents. Everyone else, including grandparents, brothers and sisters and spouses, needs a consent letter from the person named, or from that person's guardian, signed in front of a notary.
Juneau and Anchorage
Health Analytics and Vital Records runs two walk-in offices, on Commercial Boulevard in Juneau and on Old Seward Highway in Anchorage, and both are state offices, so either can issue a certificate for a birth anywhere in Alaska. The Municipality of Anchorage points residents to the state form rather than issuing copies itself. The state's records begin in the 1890s, but many births before 1930 were never registered.
What the form needs, and accepted ID
The request form asks for the full name on the record, the date of birth, the city or village of birth, and Parent A's and Parent B's names before marriage, plus your relationship, purpose and contact details. Requests without a signature and a copy of ID are not processed. Alaska accepts a driver's license or state ID, a passport, a military ID, a tribal or BIA card with a photo, or a school ID, and takes an ID that expired less than a year ago. Faxed legal documents are refused.
Heirloom certificates and public records
Alaska sells an heirloom birth certificate in two artist designs. It is a keepsake rather than a legal document, and only the person named, or a parent for a child, may order it. Birth records become public a hundred years after the birth, and older records can be supplied as a plain public record copy.
Adoptees, paternity and changes
After an adoption the state creates a new certificate, and the original opens only under the adoptee-access statute or by superior court order; adoptees eighteen or older can ask the state registrar for an uncertified copy of their original birth certificate. A father is added by acknowledgment, by affidavits or by a court, and legitimation produces a new certificate. Amendments and court-ordered name changes go to the Special Services Unit with original or certified documents, and births registered twelve or more years late become sworn delayed certificates of birth.
How it works when you order with BirthChek
Our Alaska form covers parents, guardians, attorneys, holders of a power of attorney and people with the registrant's notarized consent. If you are ordering with a consent letter, your order page asks you to upload it before we file. You give both parents' names before marriage and the city or village of birth, upload your ID and sign, and BirthChek files with Alaska Health Analytics and Vital Records.
